On the frontal view, this patient was bothered by the crookedness of his nose and how large and droopy his nasal tip appeared. On profile, he was bothered by the large hump on the bridge and again, the dramatically droopy nasal tip. On the three-quarter view, he disliked the cleft in the nasal tip, the large and wide tip cartilages and the droopy tip. He wanted a smaller nose but wanted to maintain some of the ethnic uniqueness of his nose. We performed a complex primary male rhinoplasty with many carefully placed grafts to give him his ideal nose.
